My name is Jodi Bleth. I have been practicing as a Psychiatric Nurse Practitioner in Tucson since 2018 with a wide range of experience treating conditions such as depression, anxiety, panic, bipolar disorder, post-traumatic stress disorder, insomnia, ADHD, obsessive-compulsive disorder, and co-morbid medical and mental health conditions.

You will receive personalized, one-on-one care, and a medication treatment plan that best suits your needs. Here’s what you can expect.
 

To determine the most appropriate treatment for you, I will initially meet with you to complete a comprehensive psychiatric evaluation. This consists of a combination of questionnaires and a clinical diagnostic interview (which is really just a conversation about what you're experiencing & relevant history). It is common for people to feel nervous about the first meeting, but most people leave feeling relieved and hopeful. I promise to listen and work with you to decide on a personalized treatment plan consistent with your needs and preferences. After the initial visit we will determine a follow-up schedule for ongoing monitoring of treatment effectiveness.

  • Do you accept insurance?
    Yes. I accept Aetna, Banner Aetna, Blue Cross Blue Shield, United Healthcare, Meritain, and UMR commercial insurance plans.
  • Do you accept Medicare?
    No. I am not able to bill for Medicare or Medicare Advantage plans.
  • What are your self-pay fees?
    An initial psychiatric evaluation is $175. Follow-up appointments for routine medication monitoring are $90.
  • Do you complete disability and FMLA paperwork?
    Yes, if appropriate for established clients and on a case-by-case basis. There is a fee for completion of these forms.
  • Do you provide ketamine treatment?
    No. However, I plan to offer this treatment in the future, and we can discuss whether ketamine might be an appropriate treatment for you.
